Conference Registration
Please print the "Registration Form" to register for the conference. You may do so by mail or fax:

Payment should be made at the time of registration unless the Tate Museum is provided with a company or agency billing name and address.
A "Medical Release Form" must be filled out and sent int with registration/payment if you will be attending the field trip.
